Personal data that I process:
Wellness in Mind processes your personal data because you use our services.

Below you will find an overview of the personal data that are processed for the services and correct file management.
- First and last name
- Sex
- Date of birth
- Place and time of birth (in case of a reading)
- Address data
- Phone number
- E-mail address
- Information about your activities on this website

 

This website and/or service does not intend to collect data about website visitors who are younger than 16 years of age. Unless they have parental or guardian permission. However, I cannot check whether a visitor is older than 16. I therefore advise parents to be involved in the online activities of their children, in order to prevent data about children being collected without parental consent. If you are convinced that I have collected personal information about a minor without this permission, please contact me at wellnessinmind@mail.com and I will delete this information.


Wellness in Mind processes your personal data for the following purposes:
- Handling your payment
- To be able to call or e-mail you if this is necessary to be able to carry out the service
- Inform you about changes to my services and products
- To deliver goods and services to you
- To improve the website and to tailor the range of products and services to your preferences
- To process personal data that I am legally obliged to do, such as data that I need for my tax return.

 

Automated decision making:

Wellness in Mind does not make decisions based on automated processing on matters that can have (significant) consequences for people. These are decisions taken by computer programs or systems, without involving a person (for example an employee of Wellness in Mind).

How long I keep personal data:

Depending on the type of service, your data will be stored for a minimum of 3 months (readings) and a maximum of 20 years (medical data).

Sharing personal data with third parties:
Wellness in Mind never provides information to third parties about the content of the treatment without your written permission and uses the codes of conduct of the professional associations NIP and CAT. Wellness in Mind will only share your personal data at your request and/or only if this is necessary for the execution of the agreement and to comply with any legal obligations. Wellness in Mind assumes that these companies/doctors treat your data confidentially and can guarantee the same level of security for your data.

 

Cookies, or similar techniques, that I use:
Wellness in Mind uses functional, analytical and tracking cookies. A cookie is a small text file that is stored in the browser of your computer, tablet or smartphone when you first visit this website. Wellness in Mind uses cookies with a purely technical functionality. These ensure that the website works properly and that, for example, your preferred settings are remembered. These cookies are also used to make the website work properly and to be able to optimize it. On your first visit to our website we already informed you about these cookies and we asked for your permission to place them. View, modify or delete data:
You have the right to view, correct or delete your personal data. In addition, you have the right to withdraw your consent to the data processing or to object to the processing of your personal data by Wellness in Mind and you have the right to data portability. This means that you can submit a request to us to send the personal data we have about you in a computer file to you or another organization mentioned by you.
You can send a request for access, correction, deletion, transfer of your personal data or request for withdrawal of your consent or objection to the processing of your personal data to wellnessinmind@mail.com.
To ensure that the request for access has been made by you, I ask you to send a copy of your proof of identity with the request. Make your passport photo, MRZ (machine readable zone, the strip with numbers at the bottom of the passport), passport number and Citizen Service Number (BSN) black in this copy. This is to protect your privacy. I will respond to your request as soon as possible, but in any case within four weeks.

Wellness in Mind would also like to point out that you have the opportunity to file a complaint with the national supervisory authority, the Dutch Data Protection Authority. You can do that via this link.

Wellness in Mind takes the protection of your data seriously and takes appropriate measures to prevent misuse, loss, unauthorized access, unwanted disclosure and unauthorized modification.
